Diagnostic criteria for Cushing's syndrome
Exclusion of physiological causes of hypercortisolism and exogenous corticosteroid exposure
24-hour urinary free cortisol above the ULN on ≥ 2 separate measurements
Late-night salivary cortisol > 145 ng/dL [> 4 nmol/L] on ≥ 2 separate measurements
Serum cortisol > 1.8 mcg/dL [> 50 nmol/L] after 1 mg overnight dexamethasone
Cushing's syndrome is unlikely
