Revised International Society for Human and Animal Mycology (ISHAM) diagnostic criteria for allergic bronchopulmonary aspergillosis

Predisposing conditions (asthma, cystic fibrosis, chronic obstructive lung disease, bronchiectasis) or compatible clinical and radiological presentation (expectoration of mucus plugs, finger-in-glove and fleeting opacities on CXR, lung collapse)

Essential components
Aspergillus fumigatus-specific IgE ≥ 0.35 kUA/L (or a positive type 1 skin test when aspergillus fumigatus-specific IgE is not available)
Serum total IgE ≥ 500 IU/mL (< 500 IU/mL may be acceptable if all other criteria are met)
Other components
Positive Aspergillus fumigatus-specific IgG (using lateral flow assays or enzyme immunoassays)
Blood eosinophil count ≥ 500 cells/mcL (current or historical)
Thin-section chest CT consistent with allergic bronchopulmonary aspergillosis (bronchiectasis, mucus plugging, and high-attenuation mucus, which is pathognomonic of allergic bronchopulmonary aspergillosis and confirms allergic bronchopulmonary aspergillosis diagnosis even if all other criteria  are not met) or fleeting opacities on CXR consistent with allergic bronchopulmonary aspergillosis
