The American Society for Radiation Oncology (ASTRO) has published a new guideline for radiation therapy (RT) in anal squamous cell carcinoma. For localized anal cancer, definitive chemoradiation is recommended with concurrent 5-fluorouracil-mitomycin, with 5-fluorouracil-mitomycin and capecitabine-mitomycin as alternative options. The recommended RT technique is intensity-modulated radiotherapy (IMRT), including volumetric modulated arc therapy (VMAT). For selected anal margin and superficial anal canal cancers, local excision is suggested if adequate surgical margins are obtained and sphincter function can be preserved. .
